The difference between AI that guesses and AI that checks
An AI that sounds confident and an AI that’s actually right are not the same thing. The gap between those two is the reason chatbots hallucinate, make up citations, and deliver wrong answers with perfect grammar. Grounding is the fix. It’s the practice of tethering AI responses to real, verifiable information so the model stops guessing and starts checking.
What it means
Grounding is the process of connecting an AI model’s outputs to factual, external sources of information rather than letting it generate responses purely from what it learned during training. An ungrounded model answers your question by predicting what a good answer probably looks like.
A grounded model answers by looking up real data first. The most common grounding method is RAG (retrieval-augmented generation). But grounding is the broader principle. RAG is one technique. Knowledge graphs, live database lookups, and tool-based retrieval are others.
Why it matters
Research shows that RAG-based grounding alone cuts hallucinations by 42 to 68%. That’s not a small improvement. For a customer service chatbot, a legal research tool, or a medical assistant, the difference between a 30% hallucination rate and a 5% hallucination rate is the difference between useful and dangerous.
NotebookLM (which we covered this week) is the most visible consumer example. It only answers from documents you upload. If the answer isn’t in your files, it says so. That’s grounding taken to its logical extreme: the model knows nothing except what you give it, which means it can’t make things up from training data it half-remembers.
Grounding is becoming a regulatory expectation. The EU AI Act requires that high-risk AI systems demonstrate factual accuracy. Companies deploying AI in healthcare, finance, and legal services are building grounding into their systems not because it’s a nice feature, but because they’ll face liability if their AI confidently delivers wrong information. An ungrounded agent isn’t just unreliable. It’s a legal risk.
Simple example
You ask two people for directions to the airport. The first person has lived in town for 20 years and gives you directions from memory. They’re mostly right, but they forgot about the road construction on Fifth Street and the new one-way on Elm. The second person pulls up a live map on their phone, checks traffic, and gives you turn-by-turn directions based on what the roads look like right now.
The first person is an ungrounded model working from training data. The second person is a grounded model checking real sources before answering. Both sound confident. Only one checked.
